Patient-Centered and Data-Driven Solutions to Polypharmacy in Corrections

Correctional Medicine Grand Rounds are generally held on the first and third Thursday of each month at 1:00PM Eastern time. The next Grand Rounds session will take place at 1:00 PM Eastern time on Thursday, February 5, 2026.

For this session, the ACCP welcomes Britne Wakem, PharmD, who will lead a session on practical solutions to the problem of polypharmacy in corrections. This session is a follow-up on the previous session about geriatric polypharmacy presented on January 15, 2026 by Dr. Ford.

Objectives

  • Examine how culture change can pave the way for improved quality of care.
  • Describe the purpose and key features of the WaDOC Polypharmacy Dashboard.
  • Demonstrate how the dashboard presents medication compliance and adherence data.
  • Explain how providers can use tools like the dashboard to augment clinical judgement and improve patient care.
  • Highlight how the effective use of tools such as the dashboard can reduce the prevalence of polypharmacy.

Speaker Details

Dr. Britne Wakem

She is a licensed pharmacist who currently serves as a Pharmacist Supervisor for the Washington State Department of Corrections. In this role she provides leadership and support focusing on medication safety, inventory management and quality improvement initiatives that enhance patient care. Her work includes developing and maintaining robust inventory systems, monitoring financial and clinical impacts of pharmacy services for agency leadership and legislation, and collaborating with quality and research teams. 

Prior to her current position, Dr. Wakem provided clinical pharmacy services with the Washington State Department of Veterans Affairs (WDVA). While at the WDVA she conducted comprehensive chart reviews and supported interdisciplinary behavior care team reviews. 

CME credit through the American Academy of Family Physicians. Nursing, PA, and Mental Health credit is also possible through agreements between the AAFP and your professional organizations. At this time, all Correctional Medicine Grand Rounds educational sessions are free to all attendees. Grand Rounds will be for members only or a registration fee in the next few months. Please join ACCP to take advance of all the benefits.
